Intake Forms

If you're a new client, please complete the Agreement Disclosure - Intake Forms and bring them to your first therapy session.

If you chose to receive therapeutic care via Telehealth methods, also include Consent for Telehealth Services.  

  If for some reason you are unable to complete the paperwork before our first session, 

I will have copies in my office, and we will use your session time to complete the paperwork.
